If possible, could you complete the attached form and return to our Receptionist? This will help greatly with the process for administering the booster. Thanks

We have the dates for all our booster vaccine clinics; they will be Wednesday, 10/20 and Thursday, 10/21/2021 with times as follows:
10/20 1200 to 1300- Independent Living Residents in their apartments
1300 to 1400- Personal Care Residents in their apartments
1400 to 1600- Employees in the Rec Room
10/21 1200 to1300- Bridges Residents
1300 to 1500- HCC Residents in their rooms
1500 to 1600- Employees in the Rec Room

Due to the nature of the storage of this vaccine, we will not be able to do any additional times during these dates. If you cannot be present at the above times, there are many other options to obtain the booster in the community, i.e. your local pharmacy, Walmart, etc. If you need assistance, please let me know and I will help you schedule your appointment.
