Executive Director COVID-19 update 2/3/2021

We have some very exciting news to share with you today. We have collaborated with a local pharmacy to provide a vaccination clinic for our Independent Living Residents. The vaccine that will be provided is the Moderna vaccine. The clinics will be held on Tuesday Feb 16th from 12-2 PM for the first dose and March 16th from 12-2 PM for second doses.

This notice is being placed in the in house mail slots.

There is a questionnaire and consent form attached to the notice. Please complete the highlighted sections of the form if you are interested in receiving the vaccination.

Please bring the completed form and your Medicare and Insurance cards to the reception desk. We will make copies of the front and back of your cards to attach to the questionnaire/consent form.

Once we have all of the consent forms we will input the data into the pharmacy’s system and schedule each resident for the vaccination clinic.

If you have questions regarding the clinic please contact our Resident Services Manager, Craig Vinciguerra or me for information. Please do not contact the pharmacy directly. There are Moderna Fact sheets available at the front desk.
